Integrated Stroke Program

The Integrated Stroke Recovery Model is a comprehensive system across Hamilton Health Sciences involving the Hamilton General Hospital site, the Regional Rehabilitation Centre and St. Peter's Hospital. The model provides access to acute and rehabilitation stroke best practice care for stroke patients across all of HHS and our small community hospitals. The model provides a system of seamless transition of patients to the right care at the right time, designed to meet individual stroke care  needs.  Movement to the right level and location of rehab care is determined by an early (within three to five days of hospital admission), comprehensive assessment by health professionals skilled in stroke care. Based on this assessment, the patient is transferred to the next level of care without delay, providing early access to rehabilitation where appropriate. This system is unique and ensures that patients will be assessed and triaged into a stroke service level  of care which is appropriate for their care goals. The HHS Integrated Stroke Recovery model enables stroke patients the opportunity to maximize their recovery.
