Specific Project Goals

  1. To promote optimal physical wellness for well survivors of cancer through the creation of an innovative, self-sustaining, client centered community based exercise and education program that is evidence based.
  2. Establish and promote inter-agency and inter-professional collaborative care competencies and practice so as to optimize provider roles in the delivery of patient-centered seamless care across the care continuum.
  3. Advancing supportive cancer care through establishment of education, knowledge transfer and collaborative links between Hamilton Health Sciences, Juravinski Cancer Center, and the YMCA - Hamilton / Burlington / Brantford.
  4. Develop, establish and promote health professional competencies and expertise related to musculoskeletal oncology rehabilitation within Hamilton, the Hamilton Haldimand Brantford Niagara Local Health Integrated Network (LHIN 4) and the province of Ontario.

The partnership with the YMCA-Hamilton /Burlington /Brantford is an important strategy in improving accessibility to these services for cancer survivors. Cancer survivors can access this program through regular YMCA membership upon referral from their attending physician and for those in financial need, the YMCA offers membership support.

The CanWell Program aligns perfectly with the fundamental principles of a population health framework for chronic disease management. The CanWell Program makes possible the YMCA and Hamilton Heath Sciences shared partnership vision; 

  • to promote wellness through access to innovative programs that are based on the health and wellness needs of the communities served,
  • to facilitate and enable the transformation of healthcare by directly linking acute care health services with community health and wellness promotion agencies.
