Referral Information

A referral can be made by any health professional, lawyer, rehab consultant, insurance company, employer, Veteran Affairs, or Workplace Safety and Insurance Board.  

How to Determine whether a referral should be made?

Red flags that might signal the onset of chronic pain:

  • Widespread pain
  • Poor response to traditional treatment
  • Severe pain levels
  • Poor psychosocial functioning
  • Presence of co-morbid psychiatric problems
  • Poor work performace

To participate in the four-week group-based Program, patients must be able to understand and converse in English, work in groups of people, be cooperative, and be independent in self-care i.e. dressing, personal care, etc.

Please note:  to be eligible for our service, treatment costs must be authorized by an insurance company, WSIB, or other funding agency.  Services at the Chronic Pain Management Unit are not covered by OHIP.
